Community Living Coordinator – Full Time
SUMMARY OF DUTIES: Responsible for providing individuals served opportunities to participate and to contribute to the life of the community. The Coordinator position provides guidance and supports goal-setting and personal growth of individuals served using an ongoing person-centered assessment and planning process. Coordinates service and support activities of the interdisciplinary team and other internal and external stakeholders. Ensures necessary documentation required of the clinical record is completed timely and maintained according to agency policy and procedure. Provide direct supervision and training to Direct Support Professionals. Provide on call support on a rotating basis.
REQUIREMENTS: Must have valid driver’s license, good driving record and a vehicle that is insured & privately owned. Experience working with individuals with various levels of disabilities. Ability to effectively organize and manage work flow. Functional IT skills. Ability to work in a team environment, motivates others, work a flexible schedule and demonstrate sound judgment in crisis situations. Ability to read and interpret written material, write routine reports and correspondence and speak effectively before groups of individuals served, outside stakeholders or employees.
EDUCATION: Bachelor’s degree in Human Service or related field or High School Degree/Equivalency with 2 years’ experience in Human Services Field.
